Patient-Reported Experience Measures (PREMs) are questionnaires that collect information on a patient’s experience of healthcare. We are part of the NHS and working with Welsh Government to find out whether the questionnaire being developed is fit for purpose. We need as many people as possible to complete the questions to TEST the questionnaire. This will make sure the questionnaire is capable of collecting good quality information. You can complete the survey thinking about your last healthcare appointment, no matter how long ago it was. The answers you give will not be passed on to your care team or used to change NHS services. The responses are completely anonymous unless you choose to provide your email address at the end of the questionnaire. We will not pass your contact details on to anyone else or share any identifiable information.
