Does your organisation support people with a Long Term Condition?
The first meeting of the Cardiff & Vale Long Term Conditions Alliance is being held on Monday 16th June. Funded by the Big Lottery, it is part of Co-Creating Healthy Change Portfolio, a range of 10 projects that aim to help people across Cardiff and the Vale of Glamorgan influence decisions about health and well-being services and co-create change in health and social care public service delivery.
A Long Term Conditions Alliance is being established to work with organisations supporting people with a long term condition. With the help of these organisations, partners and service users, the Alliance will seek to raise awareness of living with a long term condition, engage with partnerships and influence service delivery through consultations. In order to give you the opportunity for your views and ideas on how the Alliance will look and how it can help support your work and influence future service delivery, the meeting will take the form of a workshop event. The meeting is open to anyone who would like to contribute and benefit from the work of the Alliance.
There will be also an opportunity to network over a light lunch.
