Bereavement Questionnaire – Cardiff University

The Welsh Government recognises the significant impact that bereavement has on people’s lives. It is highly individual as well as a complex experience and people respond differently to the loss associated with the death of a loved one. People’s reactions to a death are influenced by many factors such as ethnic or religious traditions; the type of relationship ended by death; the cause of death; the person’s age at death; whether the death was sudden or expected; and many others.

Our aim is for all bereavement needs to be met, however varied they may be. To do this, we must first understand what bereavement provision currently exists across Wales. As such the Welsh Government is working with the End of Life Care Board and researchers at Cardiff University to map where bereavement services are located and the type of bereavement service that they provide, be it specialist or more general bereavement support.

To help capture this information we are seeking your support by asking you to complete the bereavement questionnaire:

The accuracy of the data depends on your response. We therefore request that you participate in a timely manner.

If you have any queries in relation to the questionnaire or the wider review, please direct them to Dr Emily Harrop at [email protected]

Yours sincerely

Annwyl gyfaill

Mae Llywodraeth Cymru yn cydnabod y gall profedigaeth gael effaith sylweddol ar fywydau pobl. Mae’n brofiad cymhleth a phersonol iawn i’r unigolyn, ac mae pobl yn ymateb mewn gwahanol ffyrdd i golled sy’n gysylltiedig â marwolaeth rhywun sy’n annwyl iddynt. Mae sawl ffactor yn dylanwadu ar sut mae pobl yn ymateb i farwolaeth, fel traddodiadau ethnig neu grefyddol; y math o berthynas a ddaw i ben yn sgil y farwolaeth; beth achosodd y farwolaeth; oedran y sawl a fu farw; a oedd yn farwolaeth sydyn neu’n ddisgwyliedig; a llawer mwy.

Ein nod yw bodloni’r holl anghenion sy’n deillio o brofedigaeth, dim ots pa mor amrywiol. Er mwyn gwneud hyn, i ddechrau, mae’n rhaid i ni wybod pa ddarpariaeth sydd ar gael ledled Cymru i helpu pobl sydd wedi cael profedigaeth. Felly, mae Llywodraeth Cymru yn gweithio gyda’r Bwrdd Gofal Diwedd Oes ac ymchwilwyr ym Mhrifysgol Caerdydd i fapio lle mae gwasanaethau profedigaeth ar gael a pha fath o wasanaethau maent yn eu darparu, boed hynny’n arbenigol neu’n gymorth mwy cyffredinol mewn profedigaeth.

Er mwyn helpu i gasglu’r wybodaeth hon rydym yn gofyn am eich cefnogaeth drwy ofyn i chi lenwi’r holiadur profedigaeth:<a href="https://eur03.safelinks.protection.outlook.com/?url=https%3A%2F%2Fcardiff.onlinesurveys.ac.uk%2Fbereavement-scoping-survey-wales&data=01%7C01%7CEvansA73%40cardiff.ac.uk%7Cdd7dbaee85c34a4adc7708d6b764c82a%7Cbdb74b3095684856bdbf06759778fcbc%7C1&sdata=q4ZNIG5rF2KRjLnUPKAzgP2Jxss4yBhTCys1OrUU5jc%3D&reserved=0" target="_blank" title="Original URL: https://cardiff.onlinesurveys.ac.uk/bereavement-scoping-survey-wales Click or tap if you trust this link.” >https://cardiff.onlinesurveys.ac.uk/bereavement-scoping-survey-wales

Mae cywirdeb y data yn dibynnu ar eich ymateb chi.  Felly byddem yn gwerthfawrogi petaech yn ymateb yn brydlon.

Os oes gennych unrhyw ymholiadau am yr holiadur neu am yr adolygiad yn ehangach, anfonwch e-bost at Dr Emily Harrop: [email protected]

Yn gywir
