Social Care Wales have organised an online event, ‘Putting the pieces together – Cluster working and connections in place-based care’, taking place on Wednesday 1st May. The event is open to anyone who has an interest in community based care. More details, including information on how to register, can be found below:

‘Putting the pieces together’ – Cluster working and connections in place-based care

1 May 10:00-11:30am

Although there is a strong focus for more local delivery of care in Wales, we continue to see gaps and barriers between services and people, leaving individuals to navigate the complex systems that we create.

Dr Karen Gully, National Professional Advisor for the Strategic Programme for Primary Care will help us explore how a focus on local communities, through a ‘Cluster’ approach, could begin to bring different teams and resources together.

We’ll invite you to discuss how we may improve connections for people accessing and providing support.

Everyone with an interest in the topic is welcome to attend.
