Launch of the Cardiff and Vale of Glamorgan Transition Protocol

You are invited to attend the launch of ‘Planning for my Future’ – the Cardiff and Vale of Glamorgan transition protocol for children and young people with additional learning needs.

We have developed, in partnership with stakeholders and young people, ‘Planning for my Future’ to help us to improve the experience of young people and their families who live within Cardiff and the Vale of Glamorgan in the transition to adulthood.

The launch will be formally attended by Sally Holland, Children’s Commissioner for Wales, with celebratory stories of success from services across the region, a showcase of the ‘Planning for my Future’ animation and hearing from young people with lived experience on the impact of working together through transition.

The launch will be virtual and held on Microsoft Teams with the option to attend all or part of the event. If you would like to reserve a place and receive your Teams invitation, please contact: Cristina.Popescu@cardiff.gov.uk.
