Over the last seven years the Bevan Exemplar programme has supported over 300 different innovation projects spanning a diverse array of specialisms and professional groups.  Previous cohorts of Exemplars have demonstrated significant success and delivered demonstrable positive impact, with many of our Exemplar projects being recognised on the national and international stage and receiving awards and other accolades.

‘Driving Change in Challenging Times’ challenges applicants to develop prudent and innovative solutions to overcome issues facing the sustainable delivery of health and care services in Wales. Applications are encouraged around the following themes, aligned with Welsh Government and NHS Wales priorities:

  • Preventing Illness, Early Intervention and Supporting Care in the Community
  • New Models of Integrated Health and Social Care
  • Reducing Waste across Health and Care
  • Supporting the Recovery of Elective Care
  • Improving Care for Patients with Cancer
  • Reducing Inequalities and Transforming Mental Health Services
  • We would also welcome other applications consistent with your organisation’s priorities.

Dros y saith mlynedd diwethaf, rydym ni wedi cefnogi 300 o brosiectau enghreifftiol i ddod o hyd i atebion arloesol i heriau’r gweithle mewn iechyd a gofal. Mae gan y prosiectau hyn gyfradd llwyddiant gyfartalog o 75%. Ar hyn o bryd, mae gennym ni 4 rhaglen fyw sy’n cynnwys 84 o brosiectau ar wahân, mewn Byrddau ac Ymddiriedolaethau Iechyd ledled Cymru.

Anogir ceisiadau sy’n seiliedig ar y themâu canlynol, sy’n cyd-fynd â blaenoriaethau Llywodraeth Cymru a GIG Cymru:

  • Modelau Newydd o Ofal Iechyd a Chymdeithasol Integredig
  • Lleihau Gwastraff ar draws Iechyd a Gofal
  • Cefnogi Adferiad Gofal Dewisol
  • Gwella Gofal i Gleifion â Chanser
  • Lleihau Anghydraddoldeb a Thrawsnewid Gwasanaethau Iechyd Meddwl
