This half-day Learning Disability Wales course serves as an introduction to PCP in transition and looks at how transition can impact young people. It looks to examine how the Real Opportunities Project works with young people, families and professionals to overcome some of the difficulties faced around transition.
The course will look forward to the statutory reform and how PCP will be used within the new Individual Development Plans.
