The Cardiff Health and Social care Network meeting will include:

• Discussion on what you want from your network.
• Dementia Friendly Communities – what are they and what is being done in Cardiff and the Vale to develop them?
• Cardiff Citizen-Driven Health – an update on what has been achieved to date, what happens next and a demonstration of the ‘Shared Well’ system that has been developed.  This is your opportunity to ask questions and maybe see how you can get involved going forward.
• Social Services & Wellbeing (Wales) Act 2015 – update on progress and introducing Dewis Cymru.
• City of Cardiff Council training for people in Health and Social Care.
