This one-day Learning Disability Wales course aims to look at and try to understand how transition can impact young people, and how you can incorporate PCP into your own practice. It looks to examine how the Real Opportunities Project works with young people, families and professionals to overcome some of the difficulties faced around transition.
Through group work and discussion the course will give participants the opportunity to try out some of the PCP tools and activities and a practice using them.  It will also give you the chance to look at how to create a one page profile.
