Cardiff and Vale Bereavement Feedback Survey

Cardiff and Vale UHB Bereavement Team invites you to take part in their Bereavement Feedback Questionnaire during Grief Awareness Week. This survey gathers insights to improve bereavement support services.

Who Can Participate: Adults aged 18+ who have experienced bereavement in the last five years.
How to Participate: Complete online via the attached QR code or use the attached paper forms available in Welsh and English and send it to Faye.Protheroe@wales.nhs.uk.

The team is also available to visit community venues to discuss and promote the survey.
