The event will bring together senior leaders and frontline staff to consider a more integrated\, community-led approach to care in Wales as part of our prudent healthcare journey and the role of effective connections across people\, pathways\, systems and organisations to support NHS Wales to improve care delivery. \nThe programme highlights include: \n\nA keynote by Dr Nick Goodwin\,co-Founder and Chief Executive Officer of theInternational Foundation for Integrated Carewho will share his experience and the latest evidence about the design and delivery of integrated health and social care.\n\nDr Goodwin will draw upon integrated models of care to explore some of the key enablers including policy\, digital solutions\, clinical leadership and coproduction\, which can help to achieve a more integrated approach to health and social care in NHS Wales.   \n\nA collection of case studies from NHS Wales\, which will demonstrate how we are connecting people and systems to provide better patient experience\, improved health outcomes and a more integrated approach to care. \n\n\nA market place to provide the opportunity to network\, share good practice and make invaluable new connections with colleagues across Wales to improve quality and safety.\n\n\nA keynote by Jos de Blok\, founder and head of Buurtzorg\, the unique district nursing system in the Netherlands which has garnered international acclaim for being entirely nurse-led and cost effective.\n\nMr de Blok will explore the move from a fragmented system to one that is connected and empowered to meet the needs of an ageing population\, He will be joined on the platform by Brendan Martin from Public World- Buurtzorg’s UK partner\, who will consider how NHS Wales can learn from the model to achieve higher levels of patient satisfaction\, a reduction in the cost of care provision and a focus on self management.” \n\nAn expert panel debate to consider how we use our connections\, improvement training and clinical expertise across Wales to achieve a more integrated approach to patient care.\n\nWe are pleased to confirm that the national conference will be CPD accredited. \nA number of themed workshops will also take place on Tuesday 28 March to complement the conference. DESCRIPTION:A one-day Children in Wales course \nThe Social Services and Wellbeing Act (Wales) 2014 came into force on 6th April this year and places a new focus on services supporting the well-being outcomes of the people they are working with.  The term ‘wellbeing’ is used to refer to the quality of people’s lives\, and covers both subjective and objective aspects.  Subjective well-being focuses on soft outcomes such as how people are feeling and how they perceive their lives and personal goals.  Objective well-being focuses on the conditions which affect those feelings such as health or education which are easier to measure in hard outcomes.
