DESCRIPTION:This seminar will share examples of how public services are collaborating to deliver a hospital discharge service which provides better outcomes for individuals. \nThe Wellbeing of Future Generations Act and the Social Services and Wellbeing Acts both require public services to work together in order to deliver better services for the people of Wales. Whilst the requirements of these acts are relatively new\, there are already organisations who are working together to deliver services in new and different ways.  \nRecent reports have brought hospital discharge processes into the public eye and have highlighted the effects that delayed discharge is having on both patients’ outcomes and on the public purse.  It is widely acknowledged that staying in hospital longer than necessary is detrimental to patients’ longer term outcomes.  Working collaboratively to support individuals to move on to their next stage of care improves those outcomes and can help to prevent unnecessary returns to hospital\, which is beneficial to both the individuals and to the services.  \nDelivered in partnership with Good Practice Wales and Welsh NHS Confederation\, this seminar will provide the opportunity for attendees to hear about a range of collaborations between different organisations and sectors in order to deliver better quality services for the citizens of Wales. \nWho the seminar is for  \nThis seminar is aimed at: \n\nHealth Teams responsible for designing and delivering health pathways;\nSocial Services and Social Care teams supporting reablement;\nHospital and community based therapy teams;\nHospital and community based Mental health teams;\nResidential and nursing home managers;\nCommunity groups and third sector organisations supporting individuals.

DESCRIPTION:Your chance to help shape the findings \nBook your place here \nInvitation \nThe Equality and Human Rights Commission invites you to their conference to share the preliminary findings of their upcoming Is Britain Fairer? 2018 review\, to be published in October this year. \nIs Britain Fairer? is the “state of the nation” report assessing progress on equality and human rights across Wales\, England and Scotland since our last report in 2015 and identifying the key challenges needing to be addressed. EHRC aim to use Welsh evidence within the report to produce Is Wales Fairer? 2018 and set the national agenda for action on equality and human rights. They need your input to help shape it. \nThe event will bring together government\, civil society and business organisations that directly affect – and are directly affected by – equality and human rights issues in Wales. Together\, we will examine the emerging findings of this influential review ahead of publication and discuss how they may impact on their work\, and yours. \nThe review looks at progress and challenges in the six domains outlined in our Measurement Framework: \n\neducation\,\nwork\,\nhealth\,\nliving standards\,\njustice and personal security\, and\nparticipation.\n\nThe event will also be an opportunity for you to alert to any evidence gaps in the review and to help shape recommendations to improve equality and human rights outcomes. DESCRIPTION:The focus of the conference will be ‘Tackling violence against women in Wales’ \nA UK-wide legal framework for tackling violence against women is to be examined from the perspective of what it means for Wales at a conference in Cardiff. \nThe Istanbul Convention\, created by the Council of Europe\, is aimed at preventing violence against women and domestic violence. It has been signed by almost 40 countries. \nThe UK Government signed the Convention in 2012 but has not yet ratified it.  It is committed to ratification but says that amendments to domestic law – to take extra-territorial jurisdiction over a range of offences – are necessary before this can be done. \nThe Cardiff Conference organised by the Wales Assembly of Women\, an independent campaigning organisation\, will hear from a range of experts about how the Istanbul Convention can be embraced in Wales. \nWales Assembly Women Chair Professor Jackie Jones\, an internationally recognised expert in human rights for women\, will kick off the debate with why the Istanbul Convention is important in Wales.  \nOther speakers include Yasmin Khan\, Welsh Government National Advisor for Violence against Women\, Gender-based Violence\, Domestic Abuse and Sexual Violence and representatives from Welsh Women’s Aid and the Equality and Human Rights Commission who will look at how the framework will work on the ground. \nHistorian Dr Rachel Lock-Lewis of USW will look at What difference did the vote make? 100 years of Women in Politics in Wales. The Audrey Jones Awards for Research by Women will be presented for research that looks at Women who choose not to have children and Gender and low pay. \nThe Great Leap Forward: Embracing the Istanbul Convention in Wales to end violence against women and girls will be held at Cardiff University School of Social Sciences\, Cardiff\, CF10 3WT on Saturday 28 April\, 10am – 3.30pm.
