SUMMARY:Care Transitions Workshop
DESCRIPTION:The NHS is leading on service change and reconfiguration of child and adolescent mental health services (CAMHS) and launched the Together for Children and Young People programme in February 2015. \nA work plan of activity is currently underway and one particular work stream\, Care Transitions\, will work to ensure there is a smooth transition between CAMHS and adult mental health services.   \nTo improve meaningful engagement with young people\, families and carers\, a pack will be developed for young people on what a good transition between the services should look like. \nTo ensure the resource pack reflects the voice of the user\, two workshops are being held in May\, one in North Wales and one in South Wales.  \nYou are invited to attend to share and discuss your experience (positive or negative) of transition between the services. \nTo register your place\, please visit http://www.eventsforce.net/nliah/836/home \nIf you and/or you family or carers are interested in attending\, please ‘register’ your interest by clicking on one of the links above no later than the 22 April 2016. DESCRIPTION:This 3-hour beginners course is for volunteers and staff who are considering\, or already undertaking\, a new project or a new community facility.
